Huisartsenzorg Twente is responsible for the processing of personal data in the processes as described above and shown in this privacy statement.
For what purposes does Huisartsenzorg Twente process your data?
Huisartsenzorg Twente only processes your data for the following purposes:
- to ensure proper care of the care described above;
- conducting the (financial) administration and internal management of Huisartsenzorg Twente;
- calculating, determining and collecting fees due for the care provided to you;
- performing an audit;
- complying with legal obligations, such as the obligation to keep records and retain records;
- any complaint handling and dispute resolution;
- improving the provision of GP care and services of Huisartsenzorg Twente.
- to inform you about the general practitioner care provided by the emergency rooms;
- to be able to contact you and respond to questions you have asked; and
- to support scientific research, education and information.
What personal data does Huisartsenzorg Twente process?
- name, address and place of residence;
- date of birth;
- telephone number;
- own general practitioner;
- BSN number;
- insurance data;
- telephone conversations at the General Practitioner are recorded on tape;
- your request for help;
- data about your health and the care provided by employees of Huisartsenzorg Twente or the GP practices; and
- other information insofar as this is necessary for proper assistance to you.
Legal Bases for Processing
We only process your personal data:
- if you give your consent;
- for the execution of the treatment agreement concluded with you and for the internal management of the institution;
- to comply with legal obligations;
- to protect vital interests of yourself or others (acute emergencies);
- for the representation of legitimate interests of Huisartsenzorg Twente or a third party.
Legitimate interest
The legitimate interest of Huisartsenzorg Twente, as described above, lies in:
- to carry out quality measurements aimed at improving and evaluating the quality of the care to be provided by GP medical stations in general;
- complaint handling.
The legitimate interest of a third party could lie in conducting scientific research, for example.
If you do not want your personal data to be used on the basis of a legitimate interest in a specific case, you can object to this. How is that possible? You can read more about this below under the heading ‘Your rights’.
How do we protect your personal data?
Huisartsenzorg Twente takes appropriate technical and organisational measures to protect your personal data against loss or any form of unlawful processing. For example, Huisartsenzorg Twente has carefully selected its (software) suppliers (processors) and made clear agreements with them about confidentiality and security. In addition, it has taken various security measures, including (but not limited to) the use of the UZI card, the logging of access by persons working at or for Huisartsenzorg Twente, secure data connections, the encryption of data and two-step verification for login.
Which people (working at or for Huisartsenzorg Twente) have access to your data?
Only persons who really need your personal data for their work have (partial) access to your data and are allowed to view and process it. These are only people who:
- be directly involved in your care provision (general practitioner, triagist, physician assistant, nurse specialist or practice nurse);
- be involved in the administrative and financial settlement of the care to be provided or provided to you;
- be involved in internal quality controls;
- involved in the settlement of complaints.
Disclosure of your personal data to third parties
Your personal data is in safe hands with Huisartsenzorg Twente. Huisartsenzorg Twente does not share your personal data with anyone other than persons working at or for Huisartsenzorg Twente (third parties) unless:
- you consent to the relevant disclosure;
- you have asked to provide this data to a third party;
- Huisartsenzorg Twente is legally obliged to provide this information;
- the provision of data is necessary to provide good care to you and to be able to carry out the treatment agreement concluded with you;
- the provision of information is necessary for the management of the institution;
- the data are necessary in judicial proceedings or complaint or disciplinary proceedings;
- this data is necessary for scientific and/or statistical research and it is not reasonably possible to ask for your permission;
- the provision of your personal data is necessary to prevent serious harm to your health or that of another person.
Retention periods
Huisartsenzorg Twente adheres to the legal periods for storing your personal data. Your personal data will not be stored longer than necessary. This is how Huisartsenzorg Twente stores:
- data from your medical file: in principle twenty years;
- recordings of telephone conversations: in principle two years;
- camera images; in principle three weeks;
- declaration and administration data: in principle seven years;
- a possible complaint file: in principle up to two years after the complaint file has been closed.
Your rights
You have various rights under privacy legislation. Huisartsenzorg Twente lists the rights for you.
Right of access
You have the right to view the personal data processed by Huisartsenzorg Twente and to receive an overview of this. You should first contact your GP practice for this, because they will enter the data.
Correction law
You have the right to have personal data changed if this data is incorrect.
Right of destruction
You have the right to submit a request for destruction. In that case, Huisartsenzorg Twente will destroy your personal data unless Huisartsenzorg Twente is legally obliged to retain the data or the interest of Huisartsenzorg Twente or another third party outweighs your right to have this data destroyed. You can first contact your own GP practice with this request.
Right to object
The right to object means that you can object to the processing of personal data on the basis of your special situation if it is processed on the basis of a legitimate interest of Huisartsenzorg Twente. If you submit such a request, Huisartsenzorg Twente will stop processing this data, unless compelling and legitimate grounds outweigh your right to object.
Right to restriction
Under certain circumstances, you also have the right to restrict the processing of your personal data. In short, this means that Huisartsenzorg Twente temporarily ‘freezes’ the processing of the data. You can invoke this right in four situations:
- pending the assessment of a request for correction;
- if the data processing is unlawful, but you do not wish to be erased and instead request the restriction of the use of this personal data;
- if Huisartsenzorg Twente no longer needs the data while you still need the data for (the preparation for) a lawsuit;
- if you are awaiting the assessment of an objection.
Right to data portability
You have the right to receive the data you provide to Huisartsenzorg Twente in a common file format. This right only applies to the personal data that Huisartsenzorg Twente processes on the basis of your consent or on the basis of a medical treatment agreement concluded with you. Moreover, the right only applies to the data that Huisartsenzorg Twente already processes in a digital form.

Time limits
Huisartsenzorg Twente will respond to your request to exercise (one of) your rights within one month. In the unlikely event that the response to your request takes more time, Huisartsenzorg Twente will inform you within one month. It is possible that due to the complexity of your request or the number of requests, the response period will be extended by two months by Huisartsenzorg Twente. This means that in certain cases the response period can be up to a maximum of three months.
Identification
Huisartsenzorg Twente asks for proof of your identity for all requests. Huisartsenzorg Twente does this, among other things, to prevent Huisartsenzorg Twente from providing personal data to the wrong party or from changing or deleting the wrong personal data.
Individual consideration for each request
Huisartsenzorg Twente carefully assesses each request. There may be circumstances that make Huisartsenzorg Twente unable to comply with a certain request. Huisartsenzorg Twente will always assess each request based on the individual circumstances of the case. If Huisartsenzorg Twente is unable to comply with a particular request, Huisartsenzorg Twente will of course inform you of this.
Data Protection Officer
Huisartsenzorg Twente has appointed a data protection officer (DPO). The DPO supervises compliance with privacy legislation internally and advises Huisartsenzorg Twente on privacy legislation. Huisartsenzorg Twente would like to inform you of the following matters:
- Huisartsenzorg Twente is obliged to record careful notes of your request for help, the care offered, referral, medical or aftercare in a (medical) file at the GP emergency stations.
- If you report to the General Practitioner with an acute care question, we will collect information about you from your own GP practice. We will only do this if you have given us permission to do so. With the details of your own GP, we can treat you better. We will also send your GP an observation message about the treatment to the General Practitioner for the purpose of your medical file. The purpose of this observation message is to ensure a good transfer to your own GP. Your GP can then continue the treatment. We want to be sure that we inform the right GP practice about your treatment. That is why we check this in the Registration By Name database that is managed by the Foundation for Registration by Name. In this database, the GP practices themselves record who is registered with them. (see www.inschrijvingopnaam.nl). This database does not contain any medical data about you, only that you are registered with your GP practice. If you have any questions about the database or about the use of your data, you can contact Stichting ION via the contact form on the website: www.inschrijvingopnaam.nl/contactformulier.
- All persons working at or for Huisartsenzorg Twente are obliged to treat your personal data confidentially.
- To improve the quality of care and services provided by the GP emergency stations, Huisartsenzorg Twente records telephone conversations.
- For your safety and the safety of the employees of the GP medical stations, camera images of the waiting room are recorded.
- Huisartsenzorg Twente uses cookies on the websites. A cookie is a simple small file that is sent along with pages from the website and stored by your browser on your computer’s hard drive. The information stored in it can be sent back to the servers of Huisartsenzorg Twente or those of the relevant third parties on a subsequent visit.
- This privacy statement does not apply to third-party websites that are connected to the website of Huisartsenzorg Twente by means of links.
