Our care providers try to provide you with the best possible service. It may happen that you are not satisfied or very satisfied with the care provided. Perhaps you have suggestions with which we can improve our services. We would like to hear from you.

What happens next?
Fill in the form as completely as possible and send it. If you choose to submit a complaint, you give permission to the independent complaints officer to handle your report.
Are you submitting the report on behalf of someone else? Please indicate this in the form. You will receive an authorization to enable the exchange of data.
In the event of a complaint, you will be contacted by the independent complaints officer within two weeks.
In the event of a suggestion for improvement , your suggestion will be reviewed and examined internally. If necessary, we will contact you to discuss your feedback with you in more detail. In this way, you help us to further improve our care and services.

If you believe that your complaint has not been resolved to your satisfaction, you can submit this to the disputes committee to which our GP emergency stations are affiliated: Stichting Klachten & Geschillen Eerstelijnszorg (Skge). Please contact Skge for this
